A lot of people jump on Ozempic, Wegovy or Mounjaro (tirzepatide) to lean down… then panic when they start looking smaller in clothes and in the gym.

The real question for lifters is:

Is it possible to stay muscular—or even gain muscle—while on these drugs?

Short answer: yes, but only if you treat training and nutrition like a serious athlete, not just someone on an appetite suppressant.

In our new article, we cover:

  • Why any big calorie deficit tends to pull 25–35% of weight loss from muscle
  • How GLP-1s like semaglutide and tirzepatide compare to “normal” dieting and bariatric surgery for muscle retention
  • Why Mounjaro often shows a lower percentage of muscle loss than typical diets (closer to ~20% in some data)
  • The two big levers if you want to keep or gain muscle:
    • High-intensity resistance training (3–5 times per week)
    • Dialing in protein intake (2–3 solid protein meals per day at 40–60 g each)

We also talk about what’s realistically achievable if you combine:

  • GLP-1 therapy
  • 100–150 minutes/week of proper training
  • Structured eating instead of “I just eat less because I’m not hungry”
