The biggest mistake I see with TRT monitoring is guys reducing it to total test + estradiol and ignoring the rest of the picture.

A solid TRT follow-up, in my opinion, should include:

CBC for hemoglobin/hematocrit trends
CMP for liver/kidney markers
Lipid panel because even “mild” androgen exposure can shift HDL/LDL over time
Total T + Free T since SHBG can completely change how a number feels in the real world
Sensitive estradiol, not just standard E2
PSA if age/history warrants it
Blood pressure and ideally resting heart rate, because some guys obsess over labs and forget the cardiovascular basics

Timing matters too. A lot of people pull labs at random times and then chase numbers that don’t mean much. If you’re injecting 2x weekly or EOD, try to stay consistent with when you draw blood relative to your last injection, otherwise you’re comparing apples to oranges every time.

Also, symptoms still matter. I’ve seen guys with “perfect” paper numbers who felt like garbage, and guys with slightly non-ideal labs who felt great and performed well.
